Midwest Eye-Banks follows applicable state and federal guidelines.

Medical Standards
Midwest Eye-Banks, its divisions/subsidiaries and eligible clinical staff are accredited by the Eye Bank Association of America (EBAA) and follow EBAA Medical Standards approved by the American Academy of Ophthalmology’s Committee on Eye Banks.

For detailed information pertaining to eye tissue storage requirements and processing fees, please contact Midwest Eye-Banks. 

Before we can provide tissue for surgery, we must receive the following items from each surgeon:

  • A photocopy of the surgeon’s state medical license
  • A completed copy of the Surgeons' Questionnaire
  • A completed copy of the Invoice Information Sheet
  • A completed copy of the Tracking Method Agreement

The information provided is for Eye-Bank purposes only and is not released to others. Our request for documentation is based upon the need to comply with the Medical Standards of the Eye Bank Association of America (EBAA). Upon receipt of this documentation by the Midwest Eye-Banks, you will be eligible to request and receive tissue on behalf of your patients.
